Nuvance Health Home Health Case Manager Job Description

Under the guidance of the Clinical Nurse Manager, the RN Case Manager provides nursing services to assist patients in achieving optimal physical, mental, and social well-being. The Case Manager monitors patient outcomes, communicates with physicians and other healthcare practitioners as necessary, and provides direct skilled nursing care in accordance with physician orders.

Responsibilities:
  • Assess the physical, emotional, socio-economic needs, and environmental needs of patients and their families.
  • Provide direct skilled nursing care in accordance with physician orders and document care timely.
  • Observe, evaluate, and report to the physician the patient's physical and mental condition, changes in condition, and termination of care when goals are attained or when termination of care is required according to agency policy.
  • Provide patient, family, and caregiver counseling and education.
  • Participate in the agency quality assurance and performance improvement plan, and agency-sponsored inservice training.
  • Develop, implement, and evaluate the plan of care in partnership with the patient, representative (if any), caregiver, and physician.
  • Supervise and provide clinical direction to the LPN after the first 90 days of employment.
  • Identify patient and family needs for other home health services and initiate referrals when appropriate under the direction of the clinical nurse manager.
  • Fulfill all compliance responsibilities related to the position.
  • Maintain and model Nuvance Health Foundational Values (Personal, Imaginative, Agile, Connected).
  • Demonstrate regular, reliable, and predictable attendance.
  • Perform other duties as required.

Required: Strong research skills, clear and concise interpersonal communication skills, and knowledge of systems integration. Minimum experience: one year. Desired: Bachelor's Degree in Nursing and experience in a qualified home health agency.
