Job Summary:
We are seeking a skilled RN Case Manager to join our home health team at Western CT Home Care Inc. As a key member of our team, you will provide high-quality nursing care to patients in the comfort of their own homes.
Responsibilities:
- Assess and evaluate patients' physical, emotional, and socio-economic needs, and develop individualized care plans to meet their unique needs.
- Provide direct skilled nursing care, including wound care, medication management, and education to patients and their families.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care.
- Supervise and provide clinical direction to LPNs, as needed.
Requirements:
- Current RN licensure in the state of Connecticut.
- One year of experience in home health or a related field.
- Strong research skills and excellent communication and interpersonal skills.
Working Conditions:
This position requires the ability to work independently and as part of a team, with frequent travel to patients' homes. The successful candidate will possess strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
