Job Title: Early Intervention Case Manager

At North Bay Regional Center, we are committed to providing exceptional support services to individuals with developmental disabilities and their families. As an Early Intervention Case Manager, you will play a vital role in ensuring that children and their families receive the necessary resources and support to thrive.

About the Role:

The Early Intervention Case Manager will provide intake and ongoing assessments of infants and toddlers eligible for Early Intervention Services under California Early Start program mandates. You will assist eligible children and their families with the coordination of other resources, develop an Individualized Family Service Plan, and participate in periodic reviews of the IFSP. Additionally, you will provide social service coordination to assist families in accessing services necessary to enhance the development of the child and the capacity of the family to meet the needs of the child.

Responsibilities:
  • Conduct intake and ongoing assessments of infants and toddlers eligible for Early Intervention Services
  • Assist eligible children and their families with the coordination of other resources
  • Develop and implement Individualized Family Service Plans
  • Participate in periodic reviews of the IFSP
  • Provide social service coordination to assist families in accessing services necessary to enhance the development of the child and the capacity of the family to meet the needs of the child
Requirements:
  • Master's Degree in Social Work or a related field from an accredited university
  • Credentialed Speech Pathologist or Nurse with a PHN or PNP certification
  • Three years of experience in working with children in early intervention agencies and resources
  • Knowledge of the Spanish Language is desirable
