Community Health Worker Supervisor

NYC Health + Hospitals is seeking a highly motivated and experienced Community Health Worker Supervisor to join our team. As a key member of our Public Health Corps, you will play a critical role in empowering New Yorkers to live the healthiest life possible.
Job Summary:The Community Health Worker Supervisor will supervise a team of Community Health Workers (CHWs) performing patient-centered, community health work with adult patients in collaboration with interdisciplinary teams at multiple clinic sites in NYC H+H primary and specialty care clinics. The successful candidate will have a strong background in community health, public health, or a related field, and experience in supervising staff in a healthcare setting.
Key Responsibilities:- Supervise a team of CHWs, providing support, coaching, and facilitating problem-solving for CHWs at home or community-based sites.
- Conduct weekly 1:1 supervision meetings with each CHW to review patient caseload and provide feedback and recommendations on intervention approach.
- Facilitate weekly case conferences with the CHW team to review goal-directed interventions and work with CHW team to develop and refine patient recruitment strategies to ensure enrollment targets are met.
- Develop channels to facilitate communication between CHWs and clinic and hospital care teams; educate clinical teams on CHW program.
- Manage patient outreach list and track enrollment progress using the electronic health record (Epic) and other programs, as needed.
- Oversee CHW patient intake process aimed at developing patient-centered, goal-directed treatment plans.
- Stay up-to-date on, and ensure CHW team is informed of, evolving patient resources including hospital resources, community services, and benefit programs.
- Monitor and track program progress and effectiveness of CHWs.
- Identify and carry out CHW program quality improvement initiatives, such as direct CHW observation and patient follow-up interviews.
- One year of full-time experience in a government agency or community organization in providing community services to the public or in assisting members of the community in obtaining community services.
- Completion of an approved six month training program of combined classroom training and on-the-job experience in community liaison work, plus six months of full-time experience as described above.
- Experience supervising staff in a healthcare setting, non-profit, or community-based organization; comfort supervising staff both in-person and remotely.
- Familiarity with email, word processing and Internet usage and comfort learning new computer programs.
- Effective written and oral communication skills.
- Proficiency in speaking a language other than English, preferably Spanish, Haitian Creole, Mandarin, Bengali, Russian, Cantonese, French, Arabic, Hindi, Urdu, Albanian, or Korean.
- Four years of full-time experience in counseling, community work or community health activities in a government agency or community organization engaged in providing community services to the public, assisting members of the community in obtaining community services or maintaining liaison with schools, community organizations or other government agencies for the purpose of providing assistance and obtaining participation and support for implementation of community or public service programs.
