Job Title: Community Health Worker Field Supervisor

VillageCare is seeking a highly skilled and experienced Community Health Worker Field Supervisor to oversee and lead our Community Health Worker Team. As a key member of our team, you will be responsible for managing field staff and ensuring compliance with departmental regulations.

The ideal candidate will have a strong background in healthcare and supervisory experience, with excellent communication and organizational skills. If you are a motivated and passionate individual who is committed to delivering high-quality care, we encourage you to apply.

Key Responsibilities:
  • Supervise and manage the activities of the Community Health Worker team, ensuring effective communication and coordination across the Care Management team.
  • Conduct regular field visits to ensure that Community Health Workers are conducting visits in a timely and appropriate manner.
  • Manage schedules and coordinate with the CHW Manager to ensure key performance indicators are met.
  • Identify training needs and provide ongoing support and training to Community Health Workers.
  • Assist with the coordination and implementation of projects within the CHW department.
Requirements:
  • 3 years of supervisory experience in a healthcare setting.
  • Associate's Degree required; Bachelor's Degree preferred.
  • Strong problem-solving skills and ability to work independently.
  • Excellent organizational, communication, and interpersonal skills.
