Job Summary

We are seeking a skilled Nurse Practitioner to join our team in a Per Assessment role. As a key member of our Senior Community Care team, you will work closely with primary care physicians, nursing staff, and patients' families to provide comprehensive health assessments and support better care coordination and health outcomes.

Responsibilities
  • Collaborate with primary care physicians to provide high-quality care
  • Work with nursing staff and patients' families to ensure seamless care transitions
  • Document medical information accurately and efficiently
  • Participate in case management team meetings to discuss patient care plans
  • Develop and implement individualized care plans to improve patient outcomes
Requirements
  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
  • Active and unrestricted license in the state where you reside
  • Board certification through a national board, such as the American Academy of Nurse Practitioners or the American Nurses Credentialing Center
  • Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations
  • Ability to gain a collaborative practice agreement, if applicable in your state
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating stairs
Preferred Qualifications
  • 1+ years of clinical experience in a long-term care setting
  • Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs
  • Understanding of Geriatrics and Chronic Illness
  • Understanding of Advanced Illness and end-of-life discussions
  • Proficient computer skills, including the ability to document medical information with written and electronic medical records
