Job Title: Healthcare Group Visit Coordinator

Are you passionate about delivering exceptional patient care and making a meaningful impact in the community? We are seeking a highly motivated and organized Healthcare Group Visit Coordinator to join our team at Primary Health Care (PHC).

About the Role:

The Healthcare Group Visit Coordinator will play a vital role in facilitating group visits to maximize patient engagement in care. This position requires strong communication and interpersonal skills, as well as the ability to work effectively with patients, families, and healthcare teams.

Key Responsibilities:
  • Develop and implement non-clinical procedures and workflows related to group visits.
  • Support the Women's Health Program Manager and others with the development or evaluation of training materials to be used in group visits.
  • Conduct outreach to patients who may benefit from group visits.
  • Coordinate the logistics for group visits, including securing a confidential location, communicating details with participants and care teams, and making arrangements for virtual participation as needed.
  • Provide individualized support to patients as needed to maximize their ability to attend group visits.
  • Ensure successful group visits by addressing the needs of patients or clinicians as needed, while acting within the scope of their duties.
Requirements:
  • High School diploma or equivalent.
  • One year of experience in human services, healthcare, or a related field.
  • Effective English and Spanish verbal and written communication skills.
  • Proficiency with computers, including Microsoft Word, Excel, and PowerPoint, and ability to learn and effectively use software applications, including practice management systems and electronic medical records.
  • Ability to assist patients with data input into an online program.
  • Knowledge of community resources, including entitlement programs.
  • Ability to interact in a calm and empathetic, yet professional and efficient manner.
  • Ability to establish and maintain effective working relationships.
  • Reliable transportation to perform community activities at off-site locations.
  • Valid Iowa Driver's License and adequate automobile insurance coverage.
Preferred Qualifications:
  • Bachelor's Degree (B.A.) from a four-year college or university in health education or a related degree.
  • Experience working with marginalized populations.
  • Community health center or public health experience.
