Join our team at Community Health Centers of Burlington as a Community Health Worker, where you will play a vital role in improving patient health and well-being.

Job Summary

We are seeking a highly motivated and compassionate Community Health Worker to join our team. As a Community Health Worker, you will be embedded into our patient care teams to provide screening, follow-up, and navigation to services for mental health and substance use disorders, as well as social determinants of health (SDOH). Your goal will be to facilitate access to services and improve patient health outcomes.

Key Responsibilities
  • Provide outreach to patients identified through population health data or care team members, using phone or video calls, or face-to-face visits in the clinic, patients' home, or community.
  • Identify eligible participants through screening patient panels from the Electronic Health Record (EHR) or population health platform who may have risk factors for poor health outcomes.
  • Assist in developing a plan of care, create appropriate referrals, and manage a panel of patients to increase health outcomes and general wellness.
  • Bridge the gap between diverse patient populations served and the healthcare system, assisting patients with navigating access to internal or community resources.
  • Work with the population health team to identify patients at risk of poor outcomes due to mental health, substance use disorders, or social determinants of health.
  • Collaborate with the patient's primary care provider, social work, and nursing, as well as other treatment team providers and community service providers engaged in the patient's care.
  • Potential to take on new projects focused on health equity, addressing barriers to care, and working with other patient populations in need of support.
Requirements
  • High school diploma or equivalent.
  • Previous experience providing social services, translation services, case management services, or other client or customer services.
  • Experience with health coaching a plus.
  • Bilingual speaker/writer preferred.
  • Strong written and verbal communication skills.
  • Ability to complete all written paperwork in a timely manner.
  • Good organizational and problem-solving skills.
  • Ability to seek supervision when needed.
  • Ability to work with diverse populations.
  • Strong boundaries and ethical decision-making.
  • Willingness to work flexible hours, including evenings if necessary.
  • Ability to work as part of a multi-disciplinary team.
Working Conditions

Work may be performed in office, community, and home settings. Hours are generally during the day but may need to be flexible to accommodate patient need. Occasional evening or Saturday hours may be required.

This position requires sitting and standing, walking, driving, or access to transportation. The position also requires typing and the ability to use a keyboard and keypad on a phone.

The position requires the use of vision to see a computer screen and hear normal speech either in person or over the phone.

  • Customer Service: All CHC employees must provide high-quality customer service to both internal customers (fellow employees) and external customers (patients and business associates) on a consistent basis.
  • Confidentiality: All CHC employees must protect the confidentiality of all patients, patient care, staff/personnel information, business and proprietary information, and financial information.
  • Compliance: Employees must complete required trainings and adhere to the Code of Conduct and policies in the Employee Handbook.
