Job Title: Clinical Program Manager - Physician Assistant/Nurse Practitioner

Job Summary: We are seeking a highly skilled Clinical Program Manager to oversee the day-to-day operations of our Advanced Illness Management program and Age Friendly Health System Projects.

Key Responsibilities:

* Manage the program's operations, including staff supervision, budgeting, and resource allocation
* Develop and implement program strategies to improve patient outcomes and satisfaction
* Collaborate with cross-functional teams to ensure seamless program execution
* Analyze program data to inform decision-making and drive continuous improvement

Requirements:

* Completion of an accredited Physician Assistant or Nurse Practitioner program with current NYS licensure and registration
* Certification: ANCC or AANP or AAPA
* ACLS and BCLS certified

Preferred Qualifications:

* Knowledge of palliative care
* Palliative care certification
* 1-2 years of Physician Assistant or Nurse Practitioner experience
* Excellent interpersonal and English verbal/written communication skills
* Ability to problem-solve and influence
* Understanding of cost accounting concepts
* Strong computer proficiency, especially with MSOffice (Word, Excel, Outlook)
