Speech Therapist Job Summary

Pinnacle Home Care Inc. is seeking a dedicated and compassionate Speech Therapist to join our team of skilled home health care providers. As a Speech Therapist, you will provide high-quality Speech Therapy services to patients in their homes, working closely with our interdisciplinary team to deliver exceptional patient care.

Responsibilities:
  • Evaluate patients' level of functioning and hearing, recommending mechanisms to enhance their hearing ability.
  • Provide initial and ongoing comprehensive assessments of patients' needs, including OASIS assessments.
  • Establish and revise Speech Therapy treatment plans in consultation with physicians.
  • Assist in developing and implementing interdisciplinary client care plans as it pertains to Speech Therapy.
  • Communicate with other team members, instructing them in speech pathology techniques as needed.
  • Teach, supervise, and counsel patients and their families in the total Speech Therapy program.
  • Record and report patient data, reactions to treatment, and changes in condition to physicians.
  • Participate in periodic revisions of physician plans of treatment, keeping them informed of patient status and obtaining additional orders as necessary.
Qualifications:
  • Graduate of an accredited Speech Therapy program or equivalent education.
  • Licensed in the State of Florida as a Speech Therapist.
  • Two years of clinical experience as a Speech Therapist.
  • Current BLS Certification.
  • Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
