Home Care Case Manager

The Registered Nurse develops, implements and evaluates plans of care for specific patient populations. Coordinates and collaborates with multidisciplinary team members to facilitate integrated and comprehensive care. Possesses clinical knowledge and skills to meet standards as required by specific clinical areas.

This position's primary duties include assessing/admitting patients, rounding checks, giving medications, talking with family members, and being responsible for the overall care of your patients. - Overall patient care
Requirements:
- RN-Associate's Degree OR RN-Bachelor's Level Degree OR RN-Diploma (Non-degree)
- Basic Cardiac Life Support, Registered Nurse
- 12+ months experience required
- Previous Home Care experience preferred
