Act as a communication bridge among physicians, nursing staff, patients, and other healthcare providers to identify patient needs. Manage and document patient inquiries related to medical conditions and concerns in accordance with established departmental protocols.
Collaborate with healthcare professionals and patients to evaluate the urgency of medical issues, address inquiries regarding medications, health education, and provide medical guidance within the defined scope of practice.
Ensure timely follow-up on patient requests.
Process new prescriptions and refill requests electronically or via fax as per physician directives.
Document all prescription and refill details in the patient's medical record.
Serve as a knowledgeable resource for clinic personnel regarding medical information, patient evaluations, and nursing management of health conditions.
Assist with in-person triage as necessary, adhering to triage policies and documenting findings in the VCC SOAP note.
Qualifications:
Minimum Requirements:
Graduation from an accredited nursing program.
Current licensure in good standing with the state nursing board.
Valid CPR certification.
A minimum of five years of experience in acute care nursing and/or outpatient clinic settings; triage experience is preferred.
Comprehensive knowledge of general nursing practices, with an understanding of acute care based on licensure and scope of practice.
Essential Skills/Knowledge/Abilities:
Proven critical thinking, organizational, and leadership capabilities; effective role modeling.
Strong history-taking skills; ability to make prompt and accurate assessments and complete documentation with precision.
Excellent communication skills to interact effectively with patients, physicians, and staff.
Ability to maintain high standards of judgment, assessment accuracy, and problem-solving skills under pressure, aligned with licensure and scope of practice.
Preferred Skills/Knowledge/Abilities:
Bilingual proficiency in English and Spanish.
Experience with Electronic Practice Management and Medical Records systems, preferably NextGen.
Compensation:
Salary range of $60.00 per hour, dependent on experience.
