Job Summary:

Community Partnership of Southern Arizona is seeking a skilled Licensed Practical Nurse (LPN) to join our behavioral health team. As a LPN, you will play a vital role in providing clinical services to our members, promoting their recovery and overall well-being.

Key Responsibilities:

  • Collaborate with prescribing clinicians and clinical staff to develop and implement individualized treatment plans.
  • Provide medical services and support, including medication delivery and administration, as ordered by prescribing clinicians.
  • Conduct assessments and observations to identify members' medical needs and report findings to prescribing clinicians or clinical staff.
  • Complete necessary charting and record-keeping in accordance with agency and funding source requirements.
  • Coordinate and provide education specific to identified medical needs, including nutritional advising and support.
  • Attend and participate in clinical staffing and other meetings as required.

Requirements:

  • Current, active, and unrestricted license in the state of AZ as a Licensed Practical Nurse (LPN).
  • Minimum 2 years of experience working as an LPN, with previous experience with Electronic Health Records (EHR) systems.
  • DPS Level I fingerprint clearance, CPR, First Aid, AED certification, and a valid Arizona Driver's License.
