Job Summary

The Licensed Practical Nurse is a vital member of our healthcare team, providing direct patient care under the supervision of a registered nurse. This role requires a strong commitment to delivering high-quality patient care and working collaboratively with other healthcare professionals to achieve positive patient outcomes.

Responsibilities
  • Provide direct patient care as defined in the state's Nurse Practice Act.
  • Implement the plan of care initiated by the registered nurse, ensuring accurate and timely documentation.
  • Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care.
  • Participate in the coordination of home health services, reporting identified needs to the registered nurse and/or Assistant Director of Nursing or Director of Nursing.
  • Use equipment and supplies effectively and efficiently.
  • Participate in personal and professional growth and development.
  • Perform other duties as assigned by the registered nurse.
Requirements
  • Graduate of an accredited practical nurse or vocational nursing program.
  • Three years of nursing experience preferred, with community/home health or medical/surgical experience being a plus.
  • Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
  • Comply with accepted professional standards and practice.
  • Demonstrate good verbal and written communication, and organization skills.
  • Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order.

Possesses and maintains current CPR certification.
