Job Title: Licensed Practical Nurse

Are you a dedicated and compassionate healthcare professional looking to make a meaningful difference in the lives of others? We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Community Partnership of Southern Arizona.

Job Summary:

Under the direction of the Medical Director and Program Director, the LPN will provide clinical services to behavioral health members, including medical procedures, referrals, and charting. The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.

Responsibilities:
  • Collaborate with prescribing clinicians and clinical staff to promote member recovery.
  • Complete medical procedures and make necessary referrals, coordinating care with primary physicians and other healthcare providers.
  • Observe members' behavior patterns and medical symptoms, reporting assessments to prescribing clinicians or clinical staff for follow-up.
  • Provide medication delivery as ordered by prescribing clinicians or other healthcare providers.
  • Complete necessary charting and record-keeping in accordance with agency and funding source requirements.
  • Coordinate and/or provide education specific to identified medical needs.
  • Provide nutritional advising and support.
  • Attend and participate in appropriate meetings and clinical staffing.
  • Administer injections to members when prescribed.
  • Perform other related duties in accordance with agency growth and changes.
Qualifications:
  • Education: Completion of nursing training from an accredited school of nursing.
  • Experience: (2) years of experience working as an LPN required, with previous experience with Electronic Health Records (EHR) systems.
  • License: Current, active, and unrestricted license in the state of AZ as a Licensed Practical Nurse (LPN).
Regulatory Requirements:
  • Minimum 18 years of age.
  • DPS Level I fingerprint clearance (must possess upon hire and maintain throughout employment).
  • CPR, First Aid, AED certification, if required (must possess upon hire and maintain throughout employment).
  • Current, valid Arizona Driver's License and 39-month Motor Vehicle Report and proof of vehicle registration liability insurance to meet insurance requirements, if required.
  • Initial current negative TB test result, if required (Employer provides).
