We are seeking a skilled and compassionate Home Health Registered Nurse to join our team at Interim HealthCare. As a Home Health Registered Nurse, you will provide high-quality patient care in the comfort of their own homes, working closely with a team of healthcare professionals to ensure the best possible outcomes.
Key Responsibilities- Provide Home-Based Care: Deliver patient-centered care to individuals with illnesses, injuries, and chronic diseases, often after discharge from a hospital, rehab, or nursing facility.
- Collaborate with Healthcare Teams: Work with physicians and oversee LPNs, CNAs, and aides to provide comprehensive patient care.
- Assess and Document Patient Progress: Examine patients, conduct assessments, document progress, and report changes to their physician.
- Education and Coaching: Educate patients on their plan of care, prescribed medication, therapy, diet, and exercise, and coach family members on patient care and home safety.
- Discharge Planning: Ensure goals are met and coordinate discharge from services.
- Active RN License in Colorado
- Home Health Care Experience (helpful but not required)
- No Case Manager Responsibilities
- Graduate of an Accredited School of Nursing
- At Least 1200 Hours of Experience as an RN
- Healthcare CPR/BLS Certification (demonstration course required)
- Pass a Background Check and Drug Test (required)
