The Field Intake Nurse is a critical role within our organization, responsible for assessing a member's eligibility for our Managed Long Term Care program. This position requires a comprehensive understanding of our program and services, as well as excellent communication skills to effectively convey the benefits to our members.
Key Responsibilities- Conduct face-to-face comprehensive assessments with potential members in their place of residence, utilizing our approved assessment tool (UASNY).
- Utilize our database of record to complete all necessary documentation, ensuring compliance with our standards and state and federal guidelines.
- Communicate plan benefits clearly and effectively to prospects and referral sources.
- Complete and submit thorough and high-quality documentation in a timely manner.
- Recognize quality of care issues and escalate them appropriately, communicating effectively with coordinators and supervisors as needed.
- Maintain member confidentiality and comply with HIPAA requirements.
- Participate in regularly scheduled and other staff meetings as requested.
- Participate in quality improvement initiatives.
- Graduate of a 2-4 year accredited nursing program.
- Active and unrestricted NYS RN license.
- Strong written and verbal communication skills.
- Proficient in the use of Microsoft Office Suite tools.
- Ease with learning new database systems.
- Must possess a valid driver's license, motor vehicle insurance, and access to a motor vehicle or reliable public transportation.
- Ability to travel within assigned area.
