Role Overview

The position involves delivering individual, group, and/or family therapy to clients and their families. Responsibilities include conducting clinical assessments, providing field-based outreach services, and offering case management and advocacy support.

Key Responsibilities

  • Conduct individualized, family-centered assessments that are strengths-based and culturally competent, gathering relevant information from families, clients, and involved agencies.
  • Integrate considerations of healthcare, substance use, co-occurring disorders, employment, or housing into treatment plans as necessary.
  • Complete Service and Coordination Plans, progress notes, and other required documentation within specified timeframes, adhering to the standards set by the Department of Mental Health and funding sources.
  • Provide rehabilitative services aimed at restoring or maintaining clients' functional, daily living, and social skills, while connecting them to essential community resources.
  • Engage in outreach, advocacy, and rehabilitative services for ongoing cases and during crisis situations.
  • Facilitate case management, including gathering client information, identifying needs, and linking clients and families to appropriate resources.
  • Deliver services in the community or field, ensuring compliance with program and funding source requirements.
  • Collaborate with the treatment team to ensure coordinated care for clients and their families.
  • Demonstrate sensitivity to the cultural and linguistic needs of the clientele.
  • Foster effective interactions with internal programs, clinical/support staff, and external entities such as schools or community organizations.
  • Meet billing expectations as per Agency requirements while providing services aligned with program needs.
  • May be required to obtain LPS designation and participate in on-call rotations, responding to after-hours calls as necessary.
  • Attend and contribute to staff meetings, providing input for program development and staff training.
  • Maintain punctuality and reliable attendance.
  • Exemplify Pacific Clinics' mission and core values in all communications.
  • Communicate effectively with a diverse consumer population, promoting positive interactions with colleagues and management.
  • Perform additional duties as assigned.

Qualifications

  • Level: Registered, licensed-eligible AMFT, APCC, ASW, or Waivered Psychologist OR Newly licensed Psychologist, MFT, and LCSW with less than [number] years of post-licensure experience.
  • Level II & III: Experienced LCSW, LMFT, LPCC, or Psychologist with a minimum of [number] years (Level II) or [number] years (Level III) of post-licensure experience.
  • Strong written and verbal communication skills.
  • Proven ability to connect with a culturally diverse clientele and staff.
  • Bilingual proficiency in languages relevant to the client population is highly desirable.
  • Familiarity with Windows operating systems, MS Word, Excel, and email, with the ability to compose professional correspondence and documentation.
  • A valid California driver's license and an insurable driving record under the Clinics' liability policy are required. A Class II California driver's license may be necessary.

Physical Demands

The role requires frequent standing or sitting, and the ability to produce records and documentation in both manual and electronic formats. Regular lifting of up to [number] pounds and occasional lifting of up to [number] pounds is expected.
