Our Palliative Care Nurse Practitioner plays a vital role in delivering comprehensive palliative care, ensuring tailored treatment plans while demonstrating outstanding clinical skills and ethical judgment.
The Nurse Practitioner will provide diagnosis, treatment, consultation, and follow-up under the guidance of the Palliative Medical Director, as mandated by state regulations.
Key Responsibilities:
- Collaborate with the Palliative Care Medical Director and team members to deliver high-quality palliative care through diagnosis, treatment, and follow-up for referred patients.
- Assume responsibility for the patient's palliative care from initial assessment to discharge.
Assessment should encompass:
disease-specific changes, functional decline, nutritional and cognitive status, pain management, and the overall impact of the disease on the patient's and family's quality of life.
- Develop an individualized palliative care plan in consultation with the patient, family, and Palliative Care Team.
- Ensure timely access to necessary care as identified in the Plan of Care.
- Participate in on-call services as required, delivering competent and responsive care.
- Prescribe medications, including controlled substances, in accordance with state law and professional practice.
- Communicate clinical findings to the Palliative Medical Director regularly and reassess the effectiveness of care plans.
- Maintain effective relationships with the supportive care team and involve them as needed to achieve desired outcomes.
- Document clinical findings and care provided in the patient's medical record to support sound medical practice.
- Stay informed about Palliative Medicine Quality Measures and ensure proper documentation and reporting.
Qualifications:
Essential Skills:
- Strong leadership, interpersonal, and communication skills.
- Ability to make independent decisions and self-motivate.
- Knowledge of Palliative Medicine principles and regulations.
- Compliance with continuing education requirements.
- Proficiency in using point of care software.
Education and Experience:
- Master's degree in Nursing from an accredited institution or equivalent.
- Minimum of two years of experience as a Nurse Practitioner.
- Preferred specialty in Family or Adult Medicine.
- Experience with Part B billing.
- Ability to work autonomously in an advanced practice role.
Licenses and Certifications:
- Current RN license or equivalent as required by state law.
- Current Nurse Practitioner certification/license.
- Collaborative agreement with a supervising physician as required by state law.
- Part B Provider certification.
- Federal and state DEA licenses as required.
- ACHPN certification in Hospice and Palliative Care preferred.
- Valid driver's license and automobile insurance.
