Job Summary:
We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Emerald Coast Palliative Care. As a Palliative Care Specialist, you will play a vital role in delivering high-quality patient care and providing support to patients and their families.
Key Responsibilities:
- Assess and manage the physical and psychosocial needs of patients referred to palliative care
- Collaborate with the Palliative Care Medical Director and other team members to develop and implement individualized care plans
- Provide medical care and treatment to patients, within the scope of practice of a Nurse Practitioner
- Participate in the coordination of patient care and ensure seamless transitions between care settings
- Participate in on-call services and provide clinically competent and responsive care to patients and families
Requirements:
- Master's degree in Nursing from an accredited school of nursing
- Collaborative agreement with a supervising physician, as required by state law
- ACHPN certification in Hospice and Palliative Care (preferred)
- Nurse Practitioner specialty in Family or Adult Medicine (preferred)
- Practice experience with Part B billing (preferred)
