The Registered Nurse (RN) for the Hospice House plays a vital role in delivering compassionate care to patients and their families during challenging times. Key Responsibilities:
- Conduct thorough assessments to identify the needs of patients and their families, employing the nursing process effectively.
- Collaborate with the Interdisciplinary Group (IDG) to develop and implement a comprehensive Plan of Care tailored to each patient's unique circumstances.
- Provide palliative and supportive care, utilizing nursing interventions and educational strategies to ensure maximum comfort and quality of life.
- Maintain open lines of communication with patients, families, and healthcare professionals, reporting any changes in patient conditions promptly.
- Facilitate education for patients and families regarding disease processes, care strategies, and safety practices.
- Participate in bereavement support for families, ensuring they receive the necessary emotional and psychological assistance.
Qualifications:
- Active RN license in the state of practice or valid multistate/compact license.
- Minimum of one year of nursing experience in a hospice or inpatient setting.
- Proficiency in using Electronic Medical Records (EMR) systems is preferred.
- Strong communication skills in English, both verbal and written.
- Ability to manage the emotional challenges associated with caring for terminally ill patients.
- Flexibility in scheduling to accommodate patient needs.
Physical Requirements:
The role requires the ability to perform essential functions, including lifting, bending, and navigating various environments to provide care effectively.
