Position Overview
We are seeking a Nurse Practitioner specializing in Palliative Care to serve patients in various locations. This role involves direct reporting to the Director of Palliative Care and collaboration with the Palliative Care Medical Director and other team members to provide comprehensive palliative care services.
Key Responsibilities:
- Evaluate the physical and psychosocial conditions of assigned patients according to established Nursing Standard Operating Procedures.
- Ensure timely and accurate documentation of patient visits.
- Deliver medical care within the Nurse Practitioner’s scope of practice as required.
- Actively engage in coordinating the palliative care plan for patients.
- Participate in on-call services during evenings and weekends, providing competent and responsive care.
Qualifications:
- Master's degree in Nursing from an accredited institution or equivalent as permitted by state regulations.
- A minimum of two years of experience as a Nurse Practitioner.
- Current licensure as a registered nurse and Nurse Practitioner as mandated by state law.
- Possession of a current Nurse Practitioner certification/license.
- Ability to establish a collaborative agreement with a supervising physician or agree to enter into such an agreement with the Palliative Medical Director, as required by state law.
- Part B Provider certification and Federal DEA license are required.
- State DEA license, if applicable.
- Preferred: ACHPN certification in Hospice and Palliative Care.
- Preferred: Nurse Practitioner specialty in Family or Adult Medicine.
- Experience with Part B billing is advantageous.
- Capability to work independently in an Advanced Practice Role.
- Understanding of Palliative Medicine principles, including patient eligibility for home health or hospice care, Medicare/Medicaid regulations, and interdisciplinary processes.
- Valid driver's license and current automobile insurance.
