Job Title: Geriatric Care Specialist - Part-Time

The estimated salary for this position is $20.00 per hour, based on the industry standards for a Geriatric Care Specialist in Connecticut.

Responsibilities:
  1. Develop and implement individualized care plans to meet the unique needs of each resident.
  2. Deliver personalized support services, including assistance with daily activities, medication management, and transportation.
  3. Collaborate with healthcare teams to ensure seamless coordination of care and services.
  4. Provide education and training to residents and caregivers on topics related to geriatric care.
  5. Maintain accurate records and communicate effectively with residents, families, and healthcare teams.
Requirements:
  • Associate's degree in Healthcare or related field preferred.
  • Minimum 2 years' experience working with seniors in a healthcare setting.
  • Current certification as a Certified Nursing Assistant (CNA) or Geriatric Care Manager (GCM) a plus.
  • Excellent communication and interpersonal skills.
  • Able to work independently and as part of a team.
