Job DescriptionThe Clinical Care Specialist provides direct care to patients, working closely with an integrated care team to ensure effective and efficient delivery of primary health care services. Responsibilities include providing chronic care disease support, triaging walk-in patients, managing medication refills, and participating in case discussions and preparing for weekly multi-disciplinary team meetings.
Required Skills & Qualifications- Associates degree in Nursing from an accredited nursing school plus at least one year of clinical experience.
- Excellent communication, interpersonal, and organizational skills.
- Outpatient primary care settings experience preferred.
- Bi-lingual English/Spanish helpful.

This role requires strong nursing knowledge base and critical thinking skills, ensuring continuity of care and flexibility in cross-training throughout all health centers.
