Job Title: In-House Physical Therapist

We are seeking a highly skilled and compassionate Physical Therapist to join our in-house team at Twin Pines North Nursing and Rehab. As a Physical Therapist, you will play a vital role in providing person-centered care to our residents, helping them achieve positive outcomes and improve their quality of life.

Responsibilities:

  • Conduct thorough assessments of patients to identify their physical needs and develop personalized treatment plans.
  • Administer a range of physical therapy treatments, including exercises, stretches, and modalities, to relieve pain, improve mobility, and promote healing.
  • Collaborate with other healthcare professionals to ensure seamless care and effective communication.
  • Monitor patient progress, adjust treatment plans as needed, and provide education to patients and their families.
  • Ensure accurate and timely documentation of patient care and treatment plans.

Requirements:

  • Active and valid Texas license in Physical Therapy.
  • Experience in Skilled Nursing Facilities or Long Term Care settings preferred.
  • Current knowledge of treatment practices and modalities.
  • Ability to work effectively in a team environment and communicate with patients, families, and healthcare professionals.
