Home Care Assistant with Compassionate Touch

We are seeking a highly skilled and empathetic Home Care Aide to provide top-notch care to our patients in their homes. As a key member of our team at Freedom Home Care - MN, you will have the opportunity to make a meaningful impact on the lives of our clients and their families.
About the RoleThis is an exceptional chance to embark on a rewarding career as a Home Care Aide, where you will be responsible for delivering personalized care and support to patients with diverse needs. Your primary focus will be on assisting patients with daily activities, ensuring their comfort, safety, and overall well-being.
You will be working closely with patients, building genuine relationships, and creating a warm and welcoming environment in their homes. Your excellent communication skills will enable you to effectively engage with patients, address their concerns, and tailor your care approach to meet their unique needs.
Duties and Responsibilities- Assist Patients with Daily Activities: You will assist patients with tasks such as moving in and out of beds, wheelchairs, or automobiles, ensuring their mobility and comfort.
- Personalized Care: You will provide personal care services, including bathing, dressing, and grooming, while respecting patients' dignity and individuality.
- Maintain Patient Records: Accurately document patient care, progress, and any concerns to ensure seamless communication with supervisors and healthcare professionals.
- Build Relationships: Foster strong bonds with patients through engaging conversations, reading aloud, and other activities that promote mental stimulation and emotional well-being.
- Medication Reminders: Ensure patients take their medications on schedule, providing vital support for their health and safety.
- Engage Patients in Activities: Encourage patients to participate in exercises, games, or hobbies that stimulate their minds and bodies.
- Education: Graduation from an accredited Home Health Aide program.
- Certifications: Hold current CPR certification, and maintain any other required certifications throughout your tenure.
- Experience: One year of prior professional experience in home health or hospice care is preferred but not mandatory.
- Licenses: Possess a valid driver's license, with a reliable vehicle and proof of insurance.
We offer a competitive compensation package, estimated at around $18.50-$22.00 per hour, commensurate with experience and qualifications. This hourly rate reflects the value we place on your expertise and dedication to delivering exceptional care to our patients.
