Job Summary

The adult care specialist will be responsible for providing personalized care and support to an adult client in their home setting. The ideal candidate will possess excellent communication and interpersonal skills, with the ability to build strong relationships with clients and their families.

Key Responsibilities
  • Provide one-on-one, client-centered care and services, including personal hygiene, meal preparation, and transfer assistance.
  • Develop and implement individualized care plans that meet the unique needs and goals of each client.
  • Collaborate with healthcare professionals and other members of the care team to ensure seamless care coordination.
Requirements
  • PSS certificate preferred but not required; must enroll in PSS course upon hire and complete within 6 months.
  • One year of clinical experience in home care preferred.
  • High school diploma or equivalent.
  • Valid driver's license and reliable transportation.
  • Successful passing of background checks.
