Nurse Case Manager Position

The Kenaitze Indian Tribe is seeking a skilled Nurse Case Manager to join our team. As a key member of our healthcare team, you will be responsible for utilizing the Dene' Model of Care to assess, plan, implement, and evaluate processes for targeting health status of assigned panels.
Key Responsibilities:- Assess the healthcare needs of assigned panels by utilizing the Dene' Model of Care and healthcare management tools.
- Prioritize and advocate for the needs of our patients.
- Utilize critical thinking skills to interpret complex information and make informed decisions.
- Collaborate with all team members within the organization, including the Alaska Native Tribal Health System, and other healthcare facilities.
- Guide patients and their families to health and well-being through education and support.
- Advocate for comprehensive and collaborative patient care by participating in staff meetings, continuing education, and training activities.
- Associates degree in nursing required; Bachelors' degree preferred.
- Three years of clinical experience, with patient population health management experience preferred.
- RN license required, with Basic Life Support certification and a valid Alaska Driver License.
- Knowledge and experience working with cultural diversities.
