Job Title: Clinical Reviewer - Nurse Practitioner

Join Tapestry as a Clinical Reviewer - Nurse Practitioner and play a vital role in ensuring the highest quality of care for our patients. As a key member of our team, you will be responsible for reviewing patient data, developing care plans, and recommending modifications to current treatments.

Key Responsibilities:
  • Review patient data reports for accuracy and completeness
  • Develop care plans for treating providers to implement
  • Review progress to care plans and identify patients at risk for a decline in health
  • Support Data Quality Reviewers, LPN Reviewers, and RN Reviewers in building their clinical knowledge and clinical decision-making
  • Coach APP new hires to help them build their understanding of the role, tools, and technology
Requirements:
  • Advanced Practice Nurse practitioner education and certification
  • Licensed as an Nurse Practitioner
  • Active and non-restricted license
  • Knowledge and Skill in chronic care management and remote patient monitoring management preferred
