About the Role

We are seeking a skilled Palliative Care Navigator to join our team at Agape Care Group. As a Palliative Care Navigator, you will play a critical role in providing high-quality, patient-centered care to individuals and families facing life-limiting illnesses.

Key Responsibilities
  • Coordinate patient care through interdisciplinary collaboration to achieve optimal patient outcomes.
  • Identify and alleviate stressors during transitions and barriers to care.
  • Function as a clinical advocate and educator for patients and families.
  • Build and maintain community relationships to provide expedient and reliable access to quality care.
Requirements
  • Current state license as a LPN or MSW.
  • Minimum of 5 years of experience in a healthcare setting.
  • Three years of Hospice and/or Palliative Care experience.
  • Reliable transportation.
  • Ability to sit, stand, bend, move intermittently and lift at least 25lbs.
