Empowering Community Partnerships Coordinator

We are seeking a highly skilled Case Manager to join our Case Management Program at Queens Community House (QCH), a leading nonprofit organization dedicated to serving the vibrant and diverse population of Queens.
About the RoleThis is a Temporary position with potential for permanent placement based on performance and organizational needs. As a Case Manager, you will be responsible for managing an average caseload of 70 older adults in the Jamaica neighborhood, working in partnership with Catholic Charities Neighborhood Services.
You will provide critical support to older adults with functional impairments, helping them gain access to essential services, benefits, and entitlements to age safely at home while maintaining their quality of life. By building trusting relationships with clients and caregivers, you will make a lasting impact on the lives of those you serve.
Key Responsibilities- Client Intake & Assessment: Conduct thorough intake processes, evaluate clients' needs, and provide accurate information about available services and benefits.
- In-Home Visits: Perform in-home assessments and reassessments to determine service eligibility and ongoing needs.
- Care Planning: Develop personalized care plans that address client needs, ensuring culturally sensitive and efficient use of community resources.
- Advocacy & Coordination: Act as a liaison with external organizations, advocate for clients, and arrange for necessary services.
- Documentation: Maintain accurate and comprehensive case records in accordance with program standards.
- Education & Experience: Bachelor's degree in Social Work (BSW) and at least two years of experience working with older adults is required. Master's degree in Social Work (MSW) or a related field is preferred.
- Cultural Competency: Strong awareness of and sensitivity to the diverse needs of older adults. Understanding of communication requirements for individuals with visual or hearing impairments.
- Language Skills: Fluency in Hindi, Urdu, Bengali, French, or Creole is highly desirable.
- Technical Proficiency: Familiarity with computer applications and databases. Knowledge of the STARS system is a plus.
- Interpersonal Skills: Excellent communication, customer service, and relationship-building skills. Ability to handle sensitive information with discretion and professionalism.
