Job Overview

We are seeking a skilled professional to join our team at Apex Place, a supportive housing program in Queens. In this role, you will provide comprehensive support services to residents, including case management, referrals, and advocacy. Your goal will be to help tenants overcome barriers and achieve their goals.

About the Position

The Case Manager position requires a strong understanding of social service systems and a passion for working with vulnerable populations. You will work closely with residents to assess their needs, develop independent living plans, and connect them with community resources. Your expertise will be essential in helping tenants navigate challenges and achieve stability.

Responsibilities

  • Review documentation to establish tenant eligibility and maintain accurate records.
  • Conduct initial intakes and periodic assessments to identify resident needs.
  • Collaborate with residents to create and update independent living plans, setting short-term and long-term goals.
  • Identify and locate community resources to assist residents in achieving their goals.
  • Make referrals to external services as needed, ensuring seamless connections for residents.
  • Monitor progress through regular contact and document via progress notes.
  • Participate in case conferences to ensure effective collaboration with internal teams.
  • Follow up with residents and referral organizations regarding contact and progress.
  • Provide required information for weekly/monthly/quarterly/annual reports.

Requirements

  • Bachelor's degree (e.g., B.A., B.S.W.) and experience working with Seriously and Persistently Mentally Ill (SPMI) populations.
