The Opportunity

We are seeking a skilled and compassionate Home Health Registered Nurse to join our team. As a Home Health Registered Nurse, you will provide patient care in the home setting, utilizing your full scope of nursing skills to deliver high-quality care.

You will work closely with patients and their families, providing care planning, case management, patient education, wound care, disease management, and more. Our team utilizes a cutting-edge remote patient monitoring system, allowing for virtual visits and enhancing patient care.

As a nurse on our Home Health team, you will have the opportunity to work autonomously with a collaborative and supportive team. Our multidisciplinary team of medical professionals shares a commitment to delivering exceptional patient care and supports each other in achieving this goal.

Shift and Schedule

This position will be paid per visit and will require an on-call schedule. One weekend per month is required.

The Work

As a Home Health Registered Nurse, you will be responsible for:

  • Utilizing the nursing process to provide patient/family care
  • Coordinating patient care and consulting with other healthcare professionals
  • Facilitating patient care, anticipating patient needs, and individualizing patient care plans
  • Collaborating with medical staff and other disciplines to enhance patient care
  • Adhering to nursing policies and standards, and striving to maintain and improve quality outcomes
