Job Title: Staff RN Per Visit-Home Health RN

Job Summary:

We are seeking a skilled and compassionate Registered Nurse to join our Home Health team as a Staff RN Per Visit-Home Health RN. As a member of our team, you will provide high-quality patient care in the comfort of their own homes, utilizing your expertise in wound care, disease management, and patient education.

Responsibilities:

  • Provide comprehensive patient care in the home setting, including wound care, disease management, and patient education.
  • Collaborate with the medical staff and other disciplines to provide and enhance patient care.
  • Facilitate patient care, anticipating patient care needs, individualizing patient care, and adjusting or modifying the nursing plan of care as appropriate.
  • Adhere to all nursing policies and standards, and proactively strive to maintain and improve quality outcomes.

Requirements:

  • 1 year of experience as a Registered Nurse in a home health or hospice setting.
  • Basic Life Support certification from the American Heart Association or Red Cross.
  • Valid driver's license.
  • Bachelor's degree in Nursing.
