Client Care Coordination Supervisor

POSITION SUMMARY:
The Care Coordination Manager is responsible for overseeing a group of clients dealing with substance use challenges alongside concurrent mental health and medical issues. This role includes ensuring quality assurance and monitoring billing processes for care coordination services.
KEY RESPONSIBILITIES:
- Engage with clients to assess their individual needs effectively.
- Facilitate the enrollment of qualified clients into a Health Home, connecting them with community service providers and supporting their adherence to appointments, which may include accompanying clients to address their comprehensive needs encompassing physical and behavioral health, financial, legal, transportation, and employment.
- Collaborate with multidisciplinary providers to ensure that clients' needs are met, aiming to enhance their overall health and reduce emergency room visits and hospitalizations.
- Oversee the accurate and timely submission of all necessary documentation by Care Coordinators.
- Monitor the submission and review of billing-related documentation to ensure timely revenue collection.
- Conduct eligibility assessments for HARP (Health and Recovery Plan), develop HCBS (Home and Community Based Services) care plans, and submit these for approval to Managed Care Organizations.
WORK SCHEDULE: Full Time
SALARY RANGE: $54,000 - $58,000 annually, based on experience

QUALIFICATIONS:
EDUCATION AND EXPERIENCE:
- Master's degree in Social Work and CASAC certification preferred.
- Outstanding skills in engaging with patients and families, as well as in the assessment and coordination of resources within hospital and community networks for effective patient care management.
- Exceptional organizational, collaborative, and psychosocial assessment skills.
- Genuine interest in contributing to innovative initiatives aimed at enhancing care quality by addressing barriers to primary and specialist services, as well as preventing unnecessary Emergency Department visits and hospital admissions/readmissions.
- Proficient in MS Office applications: Word, Excel, Outlook, with the ability to learn hospital scheduling systems, electronic medical records, and specific care coordination software.
- Bilingual in English and Spanish preferred.
