The Southwest Center for HIV AIDS INC is seeking a highly skilled Medical Assistant Care Coordinator to join our team. As a key member of our healthcare team, you will play a vital role in providing comprehensive care coordination services to our patients.
Key Responsibilities- Prepare patient records for appointments and coordinate appointments for specific care and diagnostic procedures.
- Report communicable diseases to agencies as required and initiate and manage prior authorizations for medications and procedures.
- Track laboratory and diagnostic tests, review patient gaps in care reports, and ensure appropriate follow-up of required patient care.
- Complete next day follow-up visit calls to patients and assess challenges, situations, or issues presented and prioritize while assisting with resolution.
- Work with various healthcare and support staff to assure smooth and efficient patient flow and clinical operations.
- 2 or more years of experience as a Medical Assistant.
- High school diploma or completed GED.
- Certificate of completion or diploma from an approved and accredited medical assisting training program.
- Demonstrated cultural competency to professionally and respectfully serve the LGBTQIAS+ communities.
- Excellent computer skills with proficiency in Microsoft Office and EHR software.
- Highly organized, detail-oriented self-starter with excellent problem-solving skills.
- Experience with Patient Centered Medical Home models of care.
- Experience in Integrated Care settings.
- Experience working in eClinicalWorks.
- Bilingual in Spanish and English.
- Work primarily in a climate-controlled environment with minimal safety/health hazard potential.
- Must be able to work at least two evenings per week.
- Office environment, exposure to computer screens for lengthy periods of time.
- Travel outside of the Phoenix region approximately 10% of the time.
