Job Summary

The Medical Assistant Care Coordinator will work under the direction of the Southwest Center's healthcare providers and report directly to the Clinic Operations Manager. This role plays a pivotal part in comprehensive care coordination, collaborating with interdisciplinary teams to develop personalized care plans, managing referrals and prior authorizations, and ensuring patients receive the appropriate resources and support throughout their healthcare journey.

Key Responsibilities
  • Prepare for patient appointments by reviewing and updating patient records prior to patient arrival.
  • Coordinate appointments for specific care and diagnostic procedures.
  • Report communicable diseases to agencies as required.
  • Initiate and manage prior authorizations for medications and procedures.
  • Track laboratory and diagnostic tests.
  • Review patient gaps in care reports to ensure appropriate follow-up of required patient care.
  • Assess challenges, situations, or issues presented and appropriately prioritize while assisting with resolution using strong critical thinking and problem-solving skills.
Requirements
  • 2 or more years of experience as a Medical Assistant.
  • High school diploma or completed GED.
  • Graduated and received a certificate of completion or diploma from an approved and accredited medical assisting training program.
  • Demonstrated cultural competency to professionally and respectfully serve the LGBTQIAS+ communities.
  • Excellent computer skills with proficiency in Microsoft Office and EHR software.
  • Highly organized, detail-oriented self-starter with excellent problem-solving skills.
Preferred Qualifications
  • Experience with Patient Centered Medical Home models of care.
  • Experience in Integrated Care settings.
  • Experience working in eClinicalWorks.
  • Bilingual in Spanish and English.
Work Environment

Work primarily in a climate-controlled environment with minimal safety/health hazard potential. Must be able to work at least two evenings per week. Office environment, exposure to computer screens for lengthy periods of time. Travel outside of the Phoenix region approximately 10% of the time.
