Job Title: Case Manager Early Intervention Agency

We are seeking a highly skilled and dedicated Case Manager to join our team at Little Masters Development Center L. As a Case Manager, you will play a vital role in ensuring the delivery of high-quality Early Intervention services to children and families in our community.

Responsibilities:
  • Manage a caseload of children receiving Early Intervention services, ensuring compliance with all relevant regulations and guidelines.
  • Develop and implement Individualized Family Service Plans (IFSPs) that meet the unique needs of each child and family.
  • Provide ongoing support and guidance to families, ensuring they have the resources and information needed to make informed decisions about their child's care.
  • Collaborate with a multidisciplinary team of professionals, including service coordinators, therapists, and other specialists, to ensure seamless delivery of services.
  • Maintain accurate and up-to-date records, including progress reports, case notes, and other documentation.
  • Communicate effectively with families, service providers, and other stakeholders to ensure timely and efficient delivery of services.
  • Participate in ongoing training and professional development to stay current on best practices and emerging trends in Early Intervention.
Qualifications:
  • Minimum of a high school diploma or equivalent.
  • Experience working with children and families in a healthcare or social services setting.
  • Strong organizational, communication, and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Passion for serving children and families in our community.
