POSITION SUMMARY/RESPONSIBILITIES

The Care Coordination Specialist plays a vital role in supporting STAR+PLUS Members by providing both initial and ongoing assistance. This involves identifying, selecting, obtaining, coordinating, and utilizing Covered Services and additional resources to improve the Member's overall well-being, independence, community integration, and productivity potential. The Specialist assists patients and their families in overcoming personal and environmental challenges that may hinder their ability to fully benefit from medical care.

Working collaboratively with the Service Coordination team, the Specialist conducts a comprehensive evaluation of each Member's unique dynamics, needs, and preferences. This includes delivering education and health-related information to the Member, their representative, and the Member's Support Network. The role is responsible for coordinating both covered and non-capitated services for STAR+PLUS Members, addressing their physical, behavioral, functional, and psychosocial requirements.

EDUCATION/EXPERIENCE

A minimum of three years of experience in social work is preferred, along with two years in a healthcare environment. Experience in managed care or working with individuals with disabilities and vulnerable populations facing chronic or complex conditions is also preferred. Familiarity with Medicare and Medicaid HMO processes is advantageous. Additionally, three years of experience with STAR+PLUS, Nursing Facilities, or Long Term Services and Supports (LTSS) is preferred. A Master's degree in social work from an accredited institution is required.

LICENSURE

Current unrestricted state licensure from the Texas State Board of Social Work Examiners is mandatory.
