Job Summary

LHC Group is seeking a skilled Social Work Professional to join our team as a Home Health Care Specialist. As a key member of our healthcare team, you will play a vital role in providing high-quality care to our patients and their families.

Key Responsibilities
  • Assess and Treat Patient Needs

Conduct thorough assessments of patients' mental and social conditions to identify areas of need and develop effective treatment plans.

Counsel and Educate Patients and Families

Provide counseling and education to patients and their families to help them understand and manage their conditions, and to promote healthy coping mechanisms.

Collaborate with Healthcare Team

Work closely with other healthcare professionals to ensure seamless care coordination and to promote optimal patient outcomes.

Develop and Implement Care Plans

Develop and implement individualized care plans that address patients' unique needs and goals.

Requirements
  • Master's Degree in Social Work

A master's degree in social work from a Council on Social Work Education (CSWE)-accredited program is required.

Experience in Home Health Care

At least one year of experience in home health care or a related field is preferred.

Licensure and Certification

A current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation are required. Licensure as a social worker from the state board of social work is also required.
