Position Overview:
We are in search of a qualified MDS Coordinator (RN License Required) to join our dedicated team. This full-time position operates during daytime hours and is essential to our mission of providing high-quality care.
Key Responsibilities:
1. Maintain and adhere to a schedule for all MDS due dates.
2. Coordinate the completion of MDS assessments across all disciplines.
3. Review MDS data for precision and collaborate with relevant staff to ensure accuracy.
4. Complete MDS documentation and input it into the electronic system.
5. Ensure timely transmission and approval of assessments by the State.
6. Address any rejected records and prepare them for re-submission.
7. Complete Care Area Assessments (CAAs) in accordance with state regulations.
8. Develop Resident Care Plans (RCP) in collaboration with the interdisciplinary care team, tailored to the needs of each resident and within required timelines.
9. Schedule and facilitate regular RCP meetings.
10. Communicate RCP details to appropriate staff and report any noncompliance to the supervisor.
11. Collaborate with all internal and ancillary departments to ensure understanding and adherence to the RCP.
12. Perform additional duties as assigned by the Director of Nursing or Administrator.
Qualifications:
- Must be a Registered Nurse, having graduated from an accredited School of Nursing with a current, valid RN license from the North Carolina Board of Nursing.
- Must provide proof of license renewal every two years.
- Familiarity with personnel, departmental, and facility policies, as well as compliance with local, state, and federal regulations.
- Experience with MDS-RAP and Care Planning functions is essential.
- Preference for candidates with experience in RUG-IV and MDS 3.0.
