Position Overview:
We are in search of a qualified MDS Coordinator (RN License Required) to join our team on a full-time basis during daytime hours.


Key Responsibilities:

  • Maintain and adhere to a schedule for all MDS due dates.
  • Facilitate the completion of MDS assessments across all disciplines.
  • Review MDS data for precision and collaborate with relevant staff to ensure accuracy.
  • Complete MDS documentation and enter it into the electronic system.
  • Ensure timely transmission and approval of assessments by the State.
  • Address any rejected records and prepare them for resubmission.
  • Complete CAAs in accordance with state regulations.
  • Develop Resident Care Plans (RCP) in collaboration with the interdisciplinary care team, tailored to resident needs and within required timelines.
  • Schedule and conduct regular RCP meetings.
  • Communicate RCP details to relevant staff and inform supervisors of any compliance issues.
  • Collaborate with all internal and ancillary departments to ensure understanding and adherence to the RCP.
  • Perform additional duties as assigned by the Director of Nursing or Administrator.


Qualifications:

Registered Nurse with graduation from an accredited School of Nursing and a current, valid RN license from the North Carolina Board of Nursing, with proof of license renewal submitted biennially.

Ability to comprehend and adhere to personnel, departmental, and facility policies and procedures, along with local, state, and federal regulations.

Experience with MDS-RAP and Care Planning functions is required.

Preference for candidates with experience in RUG-IV and MDS 3.0.

Background checks and a drug-free workplace are mandatory.
