Position: MDS Coordinator (RN License Required)
Employment Type: Full Time, Day Shift
Role Overview:
- Ensure adherence to a structured timeline for all MDS submissions.
- Facilitate the MDS completion process across various disciplines.
- Assess MDS data for precision and collaborate with relevant personnel to ensure correctness.
- Finalize MDS documentation and enter it into the electronic system.
- Confirm that assessments are submitted and sanctioned by the State promptly.
- Address any rejected records and prepare them for resubmission.
- Complete Comprehensive Assessment Areas (CAAs) in line with state regulations.
- Develop Resident Care Plans (RCP) in conjunction with the interdisciplinary care team, tailored to residents' needs and within the designated timeframe.
- Organize and lead RCP meetings regularly and punctually.
- Communicate RCP details to relevant staff and alert supervisors of any compliance issues.
- Collaborate with all internal and external departments to ensure understanding and adherence to the RCP.
- Perform additional duties as assigned by the Director of Nursing or Administrator.
Qualifications:
- Registered Nurse with a degree from an accredited nursing program and a valid RN license from the appropriate board.
- Ability to comprehend and follow organizational policies and procedures while adhering to local, state, and federal regulations.
- Experience with MDS-RAP and Care Planning processes is essential.
- Preferred familiarity with RUG-IV and MDS 3.0.
