Position Overview

As a Hospice Registered Nurse (Day Shift), you will play a crucial role in managing patient care and ensuring a dignified experience for those facing serious health challenges.

Key Responsibilities:

  • Directly report to the Administrator or Patient Care Manager while maintaining oversight and professional management of each patient.
  • Serve as the primary communication link between the physician, patient, family, and hospice team.
  • Deliver direct care services to hospice patients, providing essential support to their families with a manageable caseload.
  • Educate caregivers on how to provide optimal care for their loved ones.
  • Collaborate with an interdisciplinary team, including LPNs, CNAs, and support staff, to ensure comfort and dignity for patients.
  • Assess patient and family needs, assisting in the development and implementation of individualized care plans.
  • Maintain accurate clinical records for all assigned patients.
  • Engage in quality assessment and performance improvement initiatives within the organization.

Candidate Profile:

The ideal Hospice Registered Nurse / RN Case Manager will possess:

  • A compassionate and empathetic approach to patient care.
  • The ability to provide care in various settings, adapting to the patient's environment.
  • Flexibility and composure in high-pressure situations.
  • A valid RN license in the state of employment or eligibility to obtain one.
  • A minimum of one year of nursing experience in hospice, home health, ICU, geriatric, or oncology settings is preferred.
  • Additional experience in areas such as Med Surg, ER, or critical care is advantageous.
  • A current driver's license and automobile insurance.
  • Current CPR certification.
