We are seeking a dedicated and compassionate Registered Nurse to join our Hospice Care team as a Case Manager. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality patient care and support to individuals and their families.
Key Responsibilities- Plan, direct, and deliver safe patient care in accordance with our organization's standards and policies.
- Collaborate with the Interdisciplinary Team to develop and implement individualized care plans that meet the unique needs of our patients.
- Conduct thorough assessments of patients' physical, emotional, and social needs, and develop strategies to address these needs.
- Provide education and support to patients and their families to promote self-care and independence.
- Document patient progress and outcomes in a timely and accurate manner.
- Graduate of an accredited nursing school with current licensure to practice in the state of operation.
- At least one year of nursing experience, with three or more years preferred.
- Experience in hospice or a similar setting is highly desirable.
- Knowledge of the hospice philosophy of care and commitment to clinical and documentation excellence.
