We are seeking a highly skilled Care Coordinator to join our Lakewood Medical Clinic team. In this role, you will be part of a clinical care team, working to enhance patient-centered care by assessing gaps in care and creating plans with the team. You will also assist patients with medication management, access to insurance, and help identify other preventive health needs.

Job Responsibilities
  • Participate in morning huddles to anticipate patient needs
  • Work with the care team to identify gaps in care and resolve them using process improvement strategies
  • Provide brief interventions at point of care to assist patients with chronic illness management
  • Advocate for patient services with community, social service, and medical providers
