Job Summary

We are seeking a skilled and compassionate LPN to join our team as a Regional Nurse in our Group Home setting. As a key member of our healthcare team, you will provide high-quality patient care and support to individuals with Developmental and Intellectual Disabilities.

Responsibilities
  • Provide person-centered care to residents, including medication administration, documentation, and coordination of daily care.
  • Collaborate with physicians, supervisors, guardians, and other healthcare professionals to ensure comprehensive care.
  • Develop and implement individualized care plans, incorporating relevant education and training.
  • Conduct quality assurance reviews and participate in on-call rotations as scheduled.
  • Attend staff and ISP meetings as medically necessary.
  • Travel to group homes within the assigned region to provide care and support.
  • Complete training to become an American Heart Basic CPR and First Aid Instructor and instruct classes as scheduled.
Requirements
  • Certificate in nursing from an accredited nursing school.
  • Licensed as LPN by the State of Ohio and registered on the State Nurse Registry.
  • Possess skills to complete LPN duties.
  • High school diploma, high school transcripts, GED, or equivalent.
  • Copy of nursing license, BLS for Healthcare Provider, American Heart or Red Cross, driver's license, two professional reference letters, and auto insurance.
