Key Responsibilities:

The LPN will operate under the supervision of the Director of Health Services or designated clinical leadership. Responsibilities include:

  • Delivering person-centered care tailored to individual needs
  • Maintaining accurate documentation and records
  • Administering medications and coordinating daily care
  • Communicating effectively with healthcare providers, guardians, and support staff
  • Providing dietary support and monitoring
  • Participating in quality assurance reviews
  • Offering on-call consultation during business hours and participating in after-hours rotations
  • Attending necessary staff and individual service plan meetings
  • Traveling to various homes within the designated region
  • Completing training to become a certified CPR and First Aid Instructor
  • Performing additional duties as assigned

Qualifications:
  • Completion of an accredited nursing program
  • Current LPN licensure in the State of Ohio
  • Ability to perform essential LPN duties
