Job Summary:

Midwest Family of Companies is seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a Regional Group Home Nurse. As an LPN, you will play a vital role in providing person-centered care to individuals with Developmental and Intellectual Disabilities in a group home setting.

Responsibilities:

  • Provide direct care to residents, including medication administration, updating orders, and day-to-day care coordination.
  • Effectively communicate with physicians, supervisors, guardians, direct support professionals, and the Midwest Programming Department.
  • Assist with dietary support and monitoring, and provide relevant education and training based on individual care plans.
  • Participate in Quality Assurance reviews and provide on-call consultation to the Midwest DSP and Programming Department during normal business hours.
  • Attend staff and ISP meetings as medically necessary, and travel to homes within the assigned region.
  • Complete training to become an American Heart Basic CPR and First Aid Instructor and instruct classes as scheduled.

Requirements:

  • Certificate in nursing from an accredited nursing school.
  • Licensed as an LPN as issued by the State of Ohio and registered on the State Nurse Registry.
  • Possess skills to complete LPN duties.
  • High school diploma, high school transcripts, GED, or equivalent.
  • Copy of nursing license, BLS for Healthcare Provider, American Heart or Red Cross, driver's license, two professional reference letters, auto insurance, and Social Security Card.
