Key Responsibilities:

The LPN will operate under the supervision of the Director of Health Services, DON, Clinical Manager, or equivalent. Responsibilities include:

  • Delivering person-centered care to residents
  • Maintaining accurate documentation
  • Administering medications and coordinating daily care
  • Communicating effectively with Physicians, Supervisors, Guardians, and Direct Support Professionals
  • Providing dietary support and monitoring
  • Ensuring compliance with care plans through relevant education and training
  • Conducting Quality Assurance reviews within the group homes
  • Offering on-call consultation during business hours and participating in after-hours on-call rotation
  • Attending necessary staff and ISP meetings
  • Traveling to assigned homes within the designated region
  • Completing training to become an American Heart Basic CPR and First Aid Instructor
  • Performing additional duties as assigned

Qualifications:
  • Certificate in nursing from an accredited institution
  • Current LPN license issued by the State of Ohio, registered on the State Nurse Registry
  • Proficient in executing LPN responsibilities

Job Type: Full-Time

  • Copy of Nursing License
  • BLS for Healthcare Provider certification from American Heart or Red Cross
  • Valid Driver's License
  • Two professional reference letters
